CORE Program

The CORE (Community Outreach, Response and Engagement) Program provides direct outreach to community members in underserved areas by partnering with Community Collaboration of Delaware to provide community-based outreach services to individuals who have experienced health disparities related to race, gender, disability, sexual orientation, housing status, and substance use related stigmas.

The New Castle City Police Department’s Outreach Specialists provide care coordination, resource linkages, and guidance to help individuals navigate social services benefits, health insurance, and other community resources such as harm reduction and treatment services. This innovative initiative is designed to help individuals identify and build on strengths, set goals, and access the appropriate health care resources.
